Our infection prevention and control practices are designed to follow applicable public-health requirements and professional dental
standards. We continuously review our procedures and take appropriate precautions to help protect our patients and team.
Before Your Appointment
To help us maintain a safe environment for everyone, we ask our patients to complete the required forms and health information before
appointment .

Please complete the following before your visit:

If you are experiencing symptoms of a contagious illness, please contact our office before coming to your appointment.
Our team will help determine whether your appointment should be rescheduled.
What We Ask From Our Patients
To help keep our office safe and comfortable for everyone, we kindly ask that you:
1. Arrive on Time
Please arrive approximately 5 minutes before your scheduled appointment. If you arrive early, please follow the instructions provided
by our reception team.
2. Practice Good Hand Hygiene
Please sanitize your hands when entering the office and follow any infection-prevention instructions provided by our team.
Masks or face coverings may be used according to current public-health guidance or when requested by our team.
3. Maintain Good Oral Hygiene
Whenever possible, please brush and floss your teeth before your appointment. This helps us provide your dental care in a clean and
comfortable environment.
4. Help Us Minimize Crowding
We may limit the number of people in the reception and waiting areas when necessary. Patients are encouraged to attend appointments
independently whenever appropriate.
If you require assistance or have special circumstances, please let our reception team know in advance.
5. Limit Personal Belongings
For your convenience and to help maintain a clean clinical environment, please bring only essential personal belongings with you.
